Dr. Murali, a pioneer and seasoned cardiovascular and thoracic specialist, is one of the few coronary surgeons having performed 100% coronary surgeries on a beating heart without using a heart-lung machine. He has performed more than 7,000 open heart surgeries, with more than 4,500 of them on beating heart for CABG (Coronary Artery Bypass surgery) with a mortality of less than 0.5%. He has only one conversion to the pump in the last 3,500 odd OPCAB (Off Pump Coronary Artery Bypass) surgeries performed. He performed the first MICS (Minimally Invasive Cardiac Surgery) in Kerala, Right Anterior Mini Thoracotomy for closure of ASD, and ever since, has been performing MICS for Mitral and Aortic Valve surgeries as well. To his credit, Dr. Murali has trained numerous surgeons and anaesthesiologists in the field of cardiac surgery across the globe.

Pioneer in Minimally Invasive Cardiac Surgery (MICS) in Kerala – performed the state's first Right Anterior Mini Thoracotomy for ASD closure and continues leading MICS for mitral and aortic valves. Over 7,000 open-heart surgeries, including 4,500 beating-heart CABGs—maintained mortality below 0.5%, with just one pump conversion in ~3,500 OPCAB cases. First heart transplant in North Kerala (2015 at MIMS); has since completed 5 transplants with excellent outcomes. Named on Kos Island, Greece, during the 18th World Congress of World Society of Cardio-Thoracic Surgeons (2008).
